This post will be funded through our Transformation Bid, and will lead on developing a sustainable support at home offer which delivers improved outcomes for residents and keeps people living well and at home for longer.
To ensure a joined up approach, this position will also provide management and leadership to a team of commissionersand commissioning support officers who are responsible for commissioning packages for homecare and providing quality assurance of providers for residential and homecare.
This is an exciting opportunity toshape the future of commissioning for Homecare, and to work closely with other partners to ensure that people are looked after in an appropriate setting.
